In a normal battery, there is a spacer which ensures that the anode (negative) and the cathode (positive), are terminals through which current flows, not touching, and this error in the manufacturing process of the battery allowed to be enough to be current path between the terminals, the typical short circuit. As an error produced in mass, the consequences are unpredictable.

Such manufacturing flaws are pointed to a typical human error. In this case, the calibration of the machines could have been done wrongly in the production of some model of these batteries, causing a defect in the internal cell separator, it could be shorter than was supposed. Nor it is out of the question potential deterioration of accidental separation of materials (by factors such as a change in ambient temperature).
